Regardless of which option you choose, an initial $75 deposit for your child's home materials is required, and the first

installment is due by the date of the first class. In some cases, due to later registration, this might result in the home

materials deposit and the first installment being due and payable at the same time.

In order to ensure your child's place in class, a $75.00 deposit is required which will be applied to the total cost of your class. Deposits are used to purchase class supplies and are non-refundable. Deposits by check must be drawn on a local bank and must clear our account before At Home materials are received. Full payment of tuition is required by the first class unless you have elected to participate in the payment plan as laid out herein. Once class is attended and a supply kit is received, you are obligated to pay the entire fee.

I understand that my child's place in class is not guaranteed until full payment is received.

* Photo Release: I do____ do not____ give permission for my child/ren to be photographed for promotional materials including web site postings. I do so without expectation of compensation & with the understanding that these photos will be used exclusively by Masterworks Studio in weekly emails to our class. I also acknowledge that Masterworks Studio may choose not to use my photo at this time, but may do so at its own discretion at a later date. I understand that no postings on the Masterworks Studio website will be done without my express permission. I also understand, however, that once my image is posted on Masterworks Studio's website, the image can be downloaded by any computer user. Therefore, I agree to indemnify and hold harmless from any claims Merri Williams or Masterworks Studio. By submitting this form, I signify that I agree to these policies.

Checks should be made payable to Masterworks Studio and mailed to 4604 Thompson Mill Road, Decatur, GA 30034-2521. For your convenience, if you prefer to pay online with a credit card, please visit our Class Schedule page on our website and find your preferred class to register. Confirmation of your child's enrollment will be sent online.
